In terms of when to move up, even if you don't go to failure, you should have a pretty good idea of, "I could do 2 more next time with this weight," or "last week I had to stop at 6 reps because 7 would have been failure, but this week, I got 8 and 9 would have been failure." Rather than worrying about how much total weight you can lift and needing so long between sets, I'd advise only taking 2 minutes (3 tops) and lift as much as you can there. As that amount increases, use this increase as a measure of progress. You're still progressing, you're still making gains, and you're not training using a 4:1 sitting on your butt:actual lifting ratio. -3X

I used a polar heart rate monitor, which calculates calories burned using weight, fitness level, heart rate, age etc. I calculated both power and hypertrophy days twice and used the average. Polar has been shown to be fairly accurate in calculating calories burned.

And the German Volume training is awesome stuff. Why not stay with PHAT since you have gained 6 lean pounds on it and just adjust it to a push/pull/ legs routine. I do believe @skullay is on a routine like that. I was thinking about doing the same.
